We! are seeking a Director
to be
responsible for development and implementation of clinical and monitoring systems to achieve
effective and efficient population management for
Providence ElderPlace Seattle Program-of-All-Inclusive-Care for the Elderly (PACE) .
In this position you will:
Create an effective and efficient design and delivery of care management programs, services, and staffing to accomplish established operational and financial objectives.
Develop a program of Clinical Excellence Specialists oversight to ensure all clinical disciplines and the Interdisciplinary Teams are utilizing Industry Best Practices for preventative care, clinical care delivery and coordination, and population management.
Effectively work with the interdisciplinary teams, including geriatricians to identify and implement best practice in PACE care.
In partnership with Medical Director, oversees team quality an! d utilization performance.
Create and oversee system ! for implementation of health plan authorizations in accordance with industry standards.
Maintain up-to-date knowledge or current and emerging information regarding case management, PACE, Medicare/Medicaid coverage, related legislation, regulations, and market activity.
Develop strategies to manage pharmacy utilization across the continuum of care.
Facilitate the development of service delivery systems to support management of PACE participants in their community-home-based setting of choice.
Required qualifications for this position include:
Bachelor’s degree is in Nursing or other healthcare field, or a Masters in Social Work
Three to five years of progressively more-responsible experience in healthcare management
Demonstrated skills in healthcare management and strategic planning; skills and knowledge of case management, managed care, and demonstrated ability to develop clinical care delivery processes
Proven ability to build effective and professional relationships with a wide variety of people at all levels of the organization, both within and outside of the Providence system
Proficient in using computers and Microsoft Office Suite programs
Current CPR certification or able to obtain within 30 days of hire
Must have automobile, driver’s license and auto insurance
Preferred qualifications for this position include:
Master's degree in a health-related field, including Nursing, Health Care Administration, or Public Health
Experience managing services for older adults, particularly elders who require assistance with activities of daily living and have complex and chronic medical needs
Knowledge of home health, home care, hospice, and/or community-based health care services management preferred. Strong working knowledge of PACE
